Las listas de que trata el artículo anterior serán presentadas con indicación de los nombres completos de sus integrantes, su identificación, el nombre de la empresa o empleador a quien se encuentren vinculados laboralmente, la constancia de aceptación de su inclusión en la lista y la certificación de su vinculación laboral expedida por el empleador y de afiliación de éste a la Caja de que se trate. Se adicionarán las copias de las hojas de servicios respectivas.
