Datos mínimos del formulario de declaración anual. Las Superintendencias Bancaria y de Salud, de acuerdo con sus respectivas competencias, adoptarán el formulario de declaración anual de Ingreso Base de Cotización para trabajadores independientes. El formulario deberá integrar la información relativa a los diferentes riesgos cuando más de uno de ellos sea administrado por una misma entidad, o en el caso previsto en el artículo 48o. del presente decreto, y será adoptado por la respectiva entidad administradora, previa información en tal sentido a la entidades de control correspondientes.
