Para el otorgamiento de la licencia de funcionamiento se requiere solicitud escrita al Director General del Instituto, hecha por el representante legal de la Institución, acompañada de los siguientes documentos: Resolución por la cual se le reconoce la personería jurídica, o certificado de vigencia de la misma, expedidos por el Ministerio de Salud. Copia auténtica de los Estatutos de la Institución.
